RACIONAL: A doença do refluxo gastroesofágico é uma das enfermidades do trato gastrointestinal mais frequentes atualmente. A fundoplicatura de Nissen-Rossetti é técnica operatória muito empregada para o tratamento cirúrgico, sendo a migração intratorácica da válvula uma das principais complicações. OBJETIVO: Avaliar a incidência da migração da fundoplicatura e seus fatores de risco. MÉTODO: Analisou-se retrospectivamente 207 prontuários de pacientes submetidos à fundoplicatura videolaparoscópica pela técnica de Nissen-Rossetti para tratamento da doença do refluxo. As variáveis analisadas foram: sexo, idade, esofagite, grau de esofagite, tamanho da hérnia, alargamento da cárdia, encurtamento esofágico. Para as variáveis quantitativas, foi considerado o teste t de Student. Para as nominais, foi considerado o teste de Qui-quadrado ou o teste exato de Fisher. Valores de p<0,05 indicaram significância estatística. RESULTADOS: Do total, 135 eram mulheres (65,22%) e 72 homens (34,78%), com média de idade de 47,43 anos. O tamanho da hérnia variou entre 2 e 6 cm. Duzentos pacientes tinham esofagite (96,62%), sendo 113 (56,50%) grau I, 75 grau II (37,50%) e 12 grau III ou IV (6%). Alargamento de cárdia e esôfago de Barrett foram vistos em 153 (73,91%) e 13 (6,28%) casos, respectivamente. Um paciente apresentava encurtamento esofágico. Dentre as mulheres, 33 (24,4%) apresentaram migração e entre os homens, apenas seis (8,3%) (p = 0,005). A idade média dos pacientes com e sem migração foi de 54,03 e 45,89 anos, respectivamente (p = 0,001). CONCLUSÃO: A incidência da migração da fundoplicatura foi de 18,8%. O gênero feminino e possuir maior idade influenciam na probabilidade dessa migração. O grau de esofagite, tamanho da hérnia e alargamento da cárdia não se mostraram fatores de risco para migração intratorácica da fundoplicatura.

BACKGROUND: Gastroesophageal reflux is the gastrointestinal tract disease most frequently find nowadays. The Nissen-Rossetti fundoplication is widely used for the surgical treatment, and intrathoracic migration of the valve is the most frequent complication. AIM: To assess the incidence of the fundoplication and its risk factors. METHODS: Were analyzed retrospectively medical records of 207 patients undergoing laparoscopic fundoplication by the Nissen-Rossetti technique for the treatment of reflux disease. The variables analyzed were: sex, age, esophagitis grade, size of the herniation, enlargement of the cardia and esophageal shortening. For quantitative variables, was considered the Student's t test. For the nominal, was considered the chi-square or Fisher's exact test. P values <0.05 were considered statistically significant. RESULTS: Of the total, 135 were women (65.22%) and 72 men (34.78%) with mean age of 47.43 years. The size of the hernia varied between 2 and 6 cm. Two hundred patients had esophagitis (96.62%) and 113 (56.50%) grade I, 75 grade II (37.50%) and 12 grade III or IV (6%). Enlargement of the cardia and Barrett's esophagus were seen in 153 (73.91%) and 13 (6.28%) cases, respectively. One patient had esophageal shortening. Among women, 33 (24.4%) showed migration and among men, only six (8.3%) (p = 0.005). The average age of patients with and without migration was 54.03 and 45.89 years, respectively (p = 0.001). CONCLUSION: The incidence of the fundoplication migration was 18.8%. The gender (female) and higher age influence the probability of migration. The degree of esophagitis, size of hernia and enlargement of the cardia were not risk factors for intrathoracic migration of the fundoplication

FUNDAMENTOS: A hanseníase é uma enfermidade infecto-contagiosa, com distribuição mundial. Apesar de curável, permanece como um problema de saúde pública, sendo o Brasil o segundo país mais endêmico do mundo. As lesões cutâneas e o acometimento neural, que levam às principais incapacidades físicas, pioram a autoestima dos doentes, levam a uma queda da qualidade de vida e interferem na vida dos mesmos. OBJETIVO: Traçar um perfil dos pacientes acometidos pela hanseníase, atendidos no Ambulatório de Dermatologia do Hospital Evangélico de Curitiba, e caracterizar os níveis de qualidade de vida da amostra na semana prévia à pesquisa, por meio do questionário Dermatology Life Quality Index. MÉTODO: Aplicação do Dermatology Life Quality Index em 22 pacientes diagnosticados com hanseníase no período de maio a outubro de 2009. RESULTADOS: Em meio aos 22 pacientes que participaram do estudo, encontraram-se 55% do sexo masculino e 45% do sexo feminino, com idade média de 50,2 anos. Os escores obtidos na aplicação do Dermatology Life Quality Index variaram de 1 a 25 pontos, sendo a média 10,23. A maioria dos pacientes (50%) apresentou escore na dimensão do grave ou muito grave; 31,8% dos pacientes, do leve ao moderado, e 18,2% dos pacientes informaram que não houve comprometimento da qualidade de vida. CONCLUSÃO: O perfil de pacientes acometidos pela hanseníase encontrado neste estudo foram homens, com idade média de 50 anos e com forma multibacilar. A aplicação do Dermatology Life Quality Index permitiu verificar que o escore de comprometimento de qualidade de vida grave ou muito grave foi predominante na amostra, tendo mais impacto nas mulheres e não apresentando diferença em relação à hanseníase pauci ou multibacilar.

BACKGROUND: Leprosy or Hansen's disease is a chronic infectious disease with worldwide distribution. Although it is a curable disease, it remains as a public health problem, with Brazil being the second most endemic country in the world. The skin lesions and neural damage, which lead to physical disabilities, affect the patients' self-esteem, cause a decrease in quality of life and interfere in the patients' lives. OBJECTIVE: Identify the profile of the patients affected by leprosy seen at the Outpatient Clinic of Dermatology of Hospital Evangélico de Curitiba and characterize the levels of quality of life of the sample in the week prior to the survey by means of the Dermatology Life Quality Index questionnaire. METHOD: Application of the Dermatology Life Quality Index to 22 patients diagnosed with leprosy in the period from May to October 2009. RESULTS: Among the 22 patients who participated in the study, 55% were male and 45% were female, with a mean age of 50.2 years. The scores obtained with the Dermatology Life Quality Index ranged from 1 to 25 points, with a mean of 10.23. Most of the patients (50%) had scores in the range severe or very severe; 31.8% of the patients obtained scores in the range mild or moderate, and 18.2% of the patients reported that there was no impairment of their quality of life. CONCLUSION: The profile of the patients affected by leprosy in this study were men, with a mean age of 50 years and presen ting the multibacillary form of the disease. The application of the Dermatology Life Quality Index questionnaire allowed us to verify that the score severe or very severe for quality of life was predominant in the sample, with higher impact on women and no significant difference concerning pauci or multibacillary leprosy.

RACIONAL: A epidemia mundial de sobrepeso e obesidade afeta aproximadamente 1,7 bilhão de pessoas, havendo aumento nos casos de obesidade mórbida de 80% para mulheres e de 400% para homens desde 1970. A síndrome metabólica é transtorno complexo relacionado com resistência à insulina e obesidade abdominal, provocando alterações no metabolismo da glicose em vários tecidos e hiperinsulinemia. OBJETIVO: Verificar as alterações nos parâmetros pré e pós-operatórios em pacientes obesos mórbidos com síndrome metabólica, submetidos a bypass gastrointestinal em Y de Roux. MÉTODO: Casuística de 74 pacientes obesos, que apresentavam síndrome metabólica e submetidos a procedimento cirúrgico. Foram avaliados, além do IMC, a relação cintura-quadril, pressão arterial, colesterol total, HDL, LDL, triglicerídeos, glicemia de jejum no pré-operatório e no 1º e 4º meses de pós-operatório. Para variáveis que apresentaram simetria na sua distribuição, foi usado o teste t de Student para amostras pareadas. Para variáveis com distribuição assimétrica, foi usado o teste não-paramétrico de Wilcoxon. Valores de P<0,05 indicaram significância estatística. RESULTADOS: Obteve-se redução no 1º mês do pós-operatório no peso de 7,96%, IMC de 8%, relação cintura/quadril de 1,35%, colesterol total de 17,95%, HDL de 14,5%, LDL de 15,31%, TAG's de 30,29% e glicemia de 9,75%; no 4º mês, observou-se redução no peso de 14,94%, IMC de 20,08%, relação cintura/quadril de 5,96%, colesterol total de 21,27%, HDL de 3,53%, LDL de 23,72%, TAG's de 29,6% e na glicemia de 10,34%. CONCLUSÃO: Os parâmetros avaliados neste estudo tiveram melhora estatisticamente significante, com exceção do HDL-colesterol.

BACKGROUND: Overweight and obesity affects approximately 1.7 billion people, with increase in morbid obesity of 80% for women and 400% for men, since 1970. Metabolic syndrome is a complex disorder associated with insulin resistance and abdominal obesity, causing changes in glucose metabolism in various tissues and hyperinsulinemia. AIM: To verify changes in the pre and post-operative parameters in morbidly obese patients with metabolic syndrome who underwent to Roux-en-Y gastrointestinal bypass. METHOD: Seventy four patients with obesity with metabolic syndrome submitted to gastrointestinal bypass Roux-en-Y were included. All of them had measured waist-hip ratio, blood pressure, total cholesterol, HDL, LDL, triglycerides and fasting glucose pre-operatively and at the first and fourth month post-operatively. RESULTS: In the first post-operative month, happened a weight reduction of 7.96%, BMI of 8%, waist-hip ratio of 1.35%, total cholesterol of 17.95%, HDL of 14.5%, LDL of 15.31%, TAG's of 30.29% and 9.75% of glucose; on the fourth month was observed a weight reduction of 14.94%, BMI of 20.08%, waist-hip ratio of 5.96%, total cholesterol of 21.27%, HDL of 3.53%, LDL of 23.72%, TAG's of 29.6% and 10.34% of glucose. CONCLUSION: All the parameters evaluated in this study had a statistic significant improvement, with the exception of HDL-cholesterol.

RACIONAL: O trauma abdominal é o sofrimento resultante de uma ação súbita e violenta por diversos agentes. Sua incidência vem aumentando e a gravidade é determinada pela lesão de estruturas vitais do abdome e pela associação com outras lesões. OBJETIVO: Identificar as causas do trauma abdominal, relacionar as vísceras digestivas mais atingidas, a existência de lesões em outras regiões e as suas relações com sexo e faixa etária. MÉTODO: Foram selecionados 34 pacientes do Sistema Único de Saúde com diagnóstico de trauma abdominal, atendidos no período de janeiro de 2005 a setembro de 2005 no Hospital Universitário Evangélico de Curitiba. A coleta dos dados foi realizada com o auxílio de um protocolo previamente elaborado. RESULTADOS: Constatou-se que 91% das vítimas eram do sexo masculino. A faixa etária mais atingida foi a terceira década. Quanto à classificação dos traumatismos, 58,82% apresentaram ferimentos abertos e 41,18% contusões. As quedas foram responsáveis por 44 % das contusões, seguidas por acidentes automobilísticos com 35%. As origens mais comuns dos ferimentos abertos foram aquelas provocadas por armas de fogo em 56% e as por armas brancas em 44% dos casos. Ocorreu lesão no intestino delgado em 31% dos ferimentos abertos, seguido de lesão hepática, cólon e rim, cada uma com 23%. Nas contusões, 60% dos pacientes sofreram ferimentos esplênicos. O tórax foi a região mais associada ao trauma abdominal (31%). CONCLUSÃO: As principais causas de trauma abdominal foram ferimentos por arma de fogo (trauma aberto) e quedas (trauma fechado). As vísceras mais atingidas foram as parenquimatosas (baço e fígado) no trauma fechado e lesão de intestinos, fígado e rins no trauma aberto. A maioria dos pacientes era do sexo masculino com predomínio da terceira década.

BACKGROUND: Abdominal trauma is the distress resulting from a sudden and violent action effected by various agents. Its incidence has been increasing, and severity is determined by injury to vital structures in the abdomen and associated injuries. AIM: To identify the causes of abdominal trauma, the most frequently injured digestive viscera, the presence of injuries in other anatomic regions and the relationship of abdominal trauma to sex and age group. METHOD: Thirty-four patients from the Sistema Único de Saúde [the public healthcare system] were selected, all diagnosed with abdominal trauma and seen from January 2005 through September 2005 at the Hospital Universitário Evangélico de Curitiba. Data collection was performed with the aid of a previously formulated protocol. RESULTS: It was found that 91% of the victims were males. The most affected age group was in its third decade of life. Regarding the classification of traumatic injuries, 58.82% presented with open trauma wounds and 41.18% with contusions. Falls accounted for 44 % of contusions, followed by traffic accidents with 35%. The most common open wounds were caused by firearms in 56% of cases and by knives in 44%. Small intestine injury occurred in 31% of the open wounds, followed by liver, colon and kidney injury, with 23% each. In contusions, 60% of the patients sustained spleen injuries. The thorax was the region most frequently associated with abdominal trauma (31%). CONCLUSION: The leading causes of abdominal trauma were gunshot wounds (penetrating trauma) and falls (blunt trauma). The most frequently injured viscera in blunt trauma were the parenchymatous ones (spleen and liver), and intestines, liver and kidneys in penetrating trauma. Most patients were males, predominantly in their third decade of life.
